Stabilisation and pre-transfer preparation

A health care worker is in an ambulance putting something away in a cupboard next to an incubator.Care given prior to the transfer of patients is an essential component of the transfer process. You can seek advice by calling Embrace on 0114 305 8200 and speaking to the transport consultant and other specialist medical staff as necessary. Feel free to use network and Embrace guidelines to assist you. 

Measures that can be helpful in stabilising and preparing a patient for a smooth transfer can be:

  • Utilising the Embrace drug chart – prescribing, making up and labelling infusions and medications accordingly.
  • Regularly recording observations – an Embrace observation chart and GCS chart may be used.
  • Utilising the Embrace intubation checklist should intubation be required.
  • Completing medical and nursing paperwork – appropriate pages will need to be photocopied and transferred with the patient.
  • Completing a badger discharge summary or transfer letter.
  • If your hospital does not automatically share radiology images with the receiving hospital then please send by PACS’
  • Obtaining a maternal blood sample for group and save with a completed form if instructed to do so.
