Transfers

Embrace refer a patient

For all trauma and acute transfer requests please call our 24 hour hotline on 0114 305 8200 and choose the appropriate option when prompted.

For planned transfers, please use the online transfer form a planned transfer booking form, which should be printed, completed, scanned and emailed to embrace.transport@nhs.net 

The referral will then be triaged on a call back and a transport plan agreed and communicated to you.

In winter 2023/24 we have added an additional team, with the support of NHS England and YAS, to complete paediatric repatriations from PICU and wards that help create level 2 and 3 critical care capacity. Please follow the normal referral process.

Transfer documentation

Transferred patients need appropriate documentation for their ongoing care. A photocopy of all recent relevant notes including the blood test results and drug prescription chart are essential for all patients.

For paediatric patients you may wish to write a letter for the accepting team.

For neonatal patients we ask that an updated Badger summary is completed.

If your hospital does not automatically share radiology images with the receiving hospital then please send by PACS.
