Embrace refer a patient

For all trauma and acute transfer requests please call our 24 hour hotline on 0114 305 8200 and choose the appropriate option when prompted.

Referral for Planned Transfers

For planned transfers, please complete our electronic referral form. This ensures all necessary information is captured to facilitate a safe and efficient transfer.

Planned transfers generally fall into one of three categories:

  • Repatriation to a hospital nearer home
  • Transfer for a test, investigation or procedure that can’t be done at the referral hospital
  • Transfer for on-going specialist care that can’t be provided at the referral hospital

Eligibility Criteria for Planned Transfers

Before completing the form, please ensure the following criteria are met:

  •  Is it the right time clinically to refer your patient for transfer?
  •  Have the parents/carers agreed to transfer?
  •  Can all aspects of essential ongoing care be continued at the receiving hospital?
  •  If this is a planned transfer for a test, investigation or procedure, is there a confirmed date/time for this to be carried out?
  •  If this is a planned transfer for ongoing specialist care, has the patient been referred to and accepted by a named consultant?

If you have answered “No” to any of these questions, please do not complete the form. Additional preparation by your team is required before the transfer can proceed.

For repatriation of complex patients we would recommend that there is a discussion between the medical teams, to agree the transfer in principle, before referring to Embrace. 

Submitting the form

  • For Planned Transfers: Use the electronic referral form to provide patient details and transfer requirements. A member of the team will call you back to get additional clinical details and confirm arrangements.
  • For Same-Day Transfers: If transfer is required on the day, please refer the patient by phone at 0114 305 8200. The same information will be required.

Transfer documentation

Transferred patients need appropriate documentation for their ongoing care. A photocopy of all recent relevant notes including the blood test results and drug prescription chart are essential for all patients.

For paediatric patients you may wish to write a letter for the accepting team.

For neonatal patients we ask that an updated Badger summary is completed.

If your hospital does not automatically share radiology images with the receiving hospital then please send by PACS.

The transferring team call their local ambulance service and request an “Emergency Interfacility Transfer for a child”
