5-11 Covid vaccination booking form

These vaccinations are being offered to children aged 5-11 years. Please do not book an appointment if your child does not meet this criteria.

If your child has tested positive for COVID-19 and is clinically extremely vulnerable (CEV) or is living with someone who is CEV, they need to wait four weeks after having a positive COVID test before receiving another vaccine. If they are not CEV, they should have 12 weeks since their last positive COVID test before receiving a vaccine.
