Our community based team provides continuing care packages to children and young people either in the family home or in education settings. Our team is made up of highly skilled nurses, nurse associates, assistant practitioners, support workers and administrative staff who together strive to provide gold standard personalised care to children and young people within the Sheffield area.
A referral into the service can be made by any professional involved in a child’s care using the continuing care pre-assessment checklist below.

Our training is tailored to meet the child or young person’s specific needs based on individualised care plans supported by their clinician.
This specialist health training is often delivered as part of a child or young person’s continuing care package. For those children and young people who are not in receipt of continuing care there will be a cost for this service. Please enquire about costings via email and we will happily discuss this with you prior to accepting your referral in.

Ryegate House is part of Sheffield Children’s. This is a fully accessible, six bedded facility which provides short breaks with 24 hour nursing care for children and young people aged 0-19 years with complex neurological health conditions. Our primary purpose is to provide opportunities for children or young people with complex health need to spend time away from their primary carers. This includes day, evening, overnight or weekend respite.
Access to the service will be via a single referral and assessment route integrated with the children’s continuing care assessments and the local authority short breaks processes.
