At Sheffield Children's Hospital, we think that we can improve the way that we organise care and appointments for Children and Young People who have complex health needs.This survey will tell us how best we can work with families to make things better.We're inviting you to do this because your child has visited more than six services over the past two years.If you'd like to know more about this work, please contact Edd Crawley on 0114 3053321 (or call the hospital switchboard and ask for him directly)There is no obligation for you to take part. The data and contact details will remain within the Trust and used for this project only. Any contribution you give will help all families who visit the Trust regularly.
