Help make a difference to children's lives
In order to apply for a placement at Sheffield Children’s Hospital NHS Fondation Trust the terms and conditions are as follows:
3.1.1 Request the placement via the Work Placement email box at workplacements@sch.nhs.uk, or in writing to Work Placement Team, Learning and Development Department, Sheffield Children’s Foundation Trust, Western Bank S10 2TH, giving details of dates, length of placement and objectives to be achieved. If the placement is for two days or more the request must be received 8 weeks before the requested placement. A placement of one or two days can be processed within 4 weeks notice.
3.1.2 If a placement is available, provide confirmation of their student status from employer / tutor / course leader.
3.1.3 For all placements of more than one day, complete a health status form (OH2) and where necessary attend the Occupational Health Department for a health clearance check. Visitors of one day or less will not usually require OH clearance, but each request will be individually assessed by the host department and Placement Co-ordinator.
3.1.4 For all placements in a clinical environment with access to children, or where the student will be left unsupervised, either:
• Provide evidence that a CRB check has been carried out by their University/College/Employer within the previous six months, or Complete a Criminal Records Bureau (CRB) form as provided by the Trust.
• From July 2010 the student will need to be registered with the Independent Safeguarding Authority (I.S.A)
• Please note that work experience and 4-days work placement students aged 18 and over will need to have a CRB check, which will have to be funded themselves. This is also applicable for applicants not in education.
3.1.5 Carry their letter of authority with them and wear photographic ID at all times during their placement. (Work Experience students visiting the Trust for less than five days will carry non-photographic Letters of Authority issued by the Learning and Development Department provided that they are supervised at all times whilst on Trust premises).
3.1.6 Complete the placement evaluation form and return to the Placement Co-ordinator at the end of the placement.
