Stabilisation and pre-transfer preparation

Care given prior to the transfer of patients can be urgent and life-saving. You may find the information available on the guidelines pages helpful in this instance, or you can seek advice by calling Embrace and speaking to the transport doctor and the intensive care consultant in conference. 

Certain interventions can make the transfer process smoother, safer, and quicker. This means that the patient can be in a specialist centre sooner.  These interventions can be found in the guideline for transport criteria. 

Other helpful interventions include: 

• Starting an Embrace observation chart
• Prescribing and labelling medications and infusions according to the Embrace drug chart  
• Completion of medical and nursing discharge paperwork 
• Photocopying medical and nursing documentation from this admission 
• X-Rays and scans copied to CD (or hard copies made) if PACS transfer unavailable. 

It is essential that a transfer letter has been written for the receiving medical team. This must ready to be taken to the receiving hospital by the transport team.
