Making a referral with Embrace

If you need to make an acute referral to Embrace, or need further specialist input, please ring the referral line on 0845 147 2472. Our call handlers will then put you in contact with the Embrace Consultant and relevant specialists if required.

For planned non-acute transfers, please use an Embrace planned transfer booking form, which can be printed, completed and faxed to Embrace. The referral will then be triaged and an appropriately skilled team despatched according to the workload at the time.

Transfer documentation

All transferred patients need appropriate documentation for their on-going care. The Embrace transfer forms can aid this process and provide a useful template for gathering information before making the referral call. 

For neonatal transfers we ask that an updated Badger summary accompanies the patient.
